Driver Pre-Qualifications:

Thank you for your interest in a driving position with our company! We are committed to providing the highest quality service to our customers WE ARE LOOKING FOR ONLY THE BEST QUALIFIED DRIVERS.

  The following is a list of minimum qualifications required by the company.

1. MUST NOT have had a DWI/DUI/OWI/OUI or drug conviction, or a positive drug or alcohol test in the last seven (7) years or any now pending.
    

2. MUST NOT have had a preventable D.O.T. recordable accident in the past three (3) years.
    

3. MUST NOT have had more than (2) preventable accidents in the past (3) years.
    

4. MUST NOT have had more than (3) moving violations in the past three (3) years and any suspensions shown on the MVR must be shown in detail.
    

5. MUST BE at least twenty-two (25) years old.
    

6. PREFER minimum education of High School Graduate or General Equivalency Diploma (GED).
    

7. MUST HAVE one (1) year verifiable tractor/trailer experience in the last three (3) years. All owner operator equipment must pass a pre lease inspection.
    

8. You will be required to pass a D.O.T. physical from a company approved medical examiner and provide a urine sample to be used for our federally mandated drug screening program. The medical examiner will determine your ability to perform the essential functions of the position as outlined on the job description. These are paid for by the company.
    

9. Must be able to read, write and speak English.
    

10. You will be subject to D.O.T. specified Random Drug and Alcohol Testing.
     

11. If employed, you must attend all safety meetings. Failure to do so may be grounds for termination of your lease or employment.
     

12. Valid CDL, class A with tanker endorsement.
     

13. Pump required for all leased equipment.

Driver Application Instructions:

1. All information on your application must be printed in ink and in your own handwriting. PRINT LEGIBLY!
    

2. Fill in all blanks, except for the area marked "For official Use Only"
   Do not alter any forms. Doing so will terminate the qualification process.
    

3. DO NOT LEAVE ANY QUESTIONS BLANK! Mark N.A. (not applicable) to any question not applying to you.
    

4. On the "EMPLOYMENT RECORD SECTION" you must list all employers, self employment, contractors, or unemployment for a FULL 10 YEARS. There must be no gaps, and all dates, phone numbers, and addresses must be complete.
    

5. Be sure to sign and date your application.
